GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
Many patients benefit from an integrated approach to their care which needs teamwork to provide self management, prevention, lifestyle interventions and treatment. With this in mind, we often work with many other allied health providers to help patients optimise their health and wellness.
Your GP can help with referrals to allied health professionals in our local area and discuss ways in which you may be able to fund these services. Our patients often see:

- diabetic educators

Having trouble taking your medication?
Taking more than one medicine can be quite confusing. If you or someone you care for wishes help optimising medication it is a good idea to speak to your doctor. Your GP can ask a pharmacist to do a home visit to perform a Domiciliary Medication Management Review (DMMR). This is particularly useful if you are taking multiple medications, doses, having side effects, difficulty understanding or taking medication. Another time patients might benefit from a review is after being discharged from hospital.
